Panelist Application

  • This form will close on Apr 21, 2017 at 7:00 PM MST.
    1. Read the Panelist Guidelines
    2. Fill in the following form and click submit.
    3. Upon submitting this form you will receive an e-mail confirmation within 5-10 minutes (usually immediately). This does not mean that your application has been accepted, just received. Once your application has been reviewed and processed, you will be contacted by Otafest staff.
    4. Admission prices for panelists/assistants are listed in the Panelist Guidelines.

  • This individual (and the panel host) will be eligible for discounted admission to the festival as described in the panelist contract.

    List of all assistants and their roles. No badges will be given to anyone not listed here. We require proof of involvement.
    Room setup will be theatre style - which means chairs only and no tables. We will try our best to accommodate these requests, however, there is no guarantee that requested items will be provided.
  • Any other technical equipment you will need. We cannot guarantee the items in your list, but we will try our best.
  • Not applicable for all events. If you require prizes, please provide an explanation as to why.
